Common attribution: the model is not capable enough, the prompt was not written well.
Through the lens of this chapter's framework, this is a direct consequence of the faithful execution characteristic. The Agent is faithful to its input. The randomness in the output comes from the ambiguity of the spec, and has limited relation to model capability. Switch to a more powerful model, and a vague spec still produces random code. The root of the problem is on the input side: whether the spec is sufficiently clear, complete, and testable. Chapter 2 will discuss how to address this through a specification system.
"Big tasks fall apart when given to AI"
Common attribution: AI can only handle simple tasks; complex work still needs a human.
This is a direct consequence of the limited processing capacity characteristic. The same Agent performs well on small tasks but collapses on large ones. The difference lies in whether the task scale exceeds the quality cliff. The issue is in the decomposition strategy: how to break large tasks into granularities the Agent can effectively handle, and how to maintain overall consistency after decomposition. This is a direct consequence of the no memory accumulation characteristic. An Agent's memory ends at the session boundary. Errors corrected in the previous session, consensus reached, and project understanding accumulated are completely reset to zero in the new session. Experience that teams accumulate through verbal communication and daily collaboration has no effect on Agents. Chapter 4 will discuss how to build cross-session knowledge persistence mechanisms.

This is a direct consequence of the high throughput characteristic. The core conflict is that output speed exceeds review bandwidth. When an Agent's daily code output equals a programmer's weekly output, the traditional human review process inevitably becomes a bottleneck. Discipline and standards are governance tools effective for humans; for Agents, what is needed is external quality governance mechanisms: adversarial verification, trust boundaries, and risk tiering.
